Why Pregnancy Needs More Than One Scan

A single ultrasound is a photograph. Three ultrasounds across pregnancy are a story.


Each scan in this plan serves a specific clinical purpose — and none of them can substitute for the others.


Scan 1 :The Anatomy and Dating Scan (Around 11–14 Weeks)

At this stage, all major foetal structures are visible for the first time. Our radiologist assesses:

 

  • Nuchal translucency — fluid at the back of the foetal neck, used to screen for chromosomal conditions including Down syndrome.
  • Nasal bone presence
  • Crown-rump length for accurate gestational dating
  • Early anatomy — brain, spine, limb buds, heart chambers visible at this stage
  • Number of foetuses and placental position

This scan is not optional. It is the window that closes. Nuchal translucency measurement is only accurate between 11 and 13 weeks 6 days. After that, the window is gone — and cannot be recreated at a later scan.


Scan 2 : The Anomaly and Growth Scan (Around 18–22 Weeks)

The most detailed scan of any pregnancy. Every major organ system is assessed. Our radiologist examines:

  • Brain and neural tube
  • Heart — four chambers, outflow tracts, rhythm
  • Spine — full length
  • Abdominal organs — stomach, kidneys, bladder
  • Limbs — all four, with bone measurements
  • Face — cleft lip screening
  • Placental position — identifying low-lying placenta before it becomes a delivery concern
  • Amniotic fluid volume
  • Foetal growth measurements — head, abdomen, femur length — plotted against standard growth charts

This is the scan that identifies the majority of structural anomalies that are detectable in pregnancy. Missing it — or doing it outside the 18–22 week window — significantly reduces its clinical value.


Scan 3 : The Foetal Wellbeing and Delivery Planning Scan (Around 32–36 Weeks)

By the third trimester, the questions change. Growth. Position. Fluid. Placenta. The fetal head is now engaging. Delivery planning begins.


Our radiologist assesses:

  • Foetal weight estimation — are growth rates tracking normally?
  • Foetal position — head down, breech, transverse
  • Placental maturity and position — has a low-lying placenta resolved?
  • Amniotic fluid index — adequate, reduced, or excess
  • Umbilical artery Doppler — blood flow from placenta to foetus, a key indicator of foetal health
  • Cervical length in relevant cases

This scan directly informs delivery decisions — normal vaginal delivery, planned caesarean, or closer monitoring before term.

What "Fetal Wellbeing" Actually Means on a Report

Most pregnant women receive a report that says "fetal wellbeing appears satisfactory" and have no idea what was actually assessed.

 

  • FWB: Foetal Wellbeing on an obstetric ultrasound report refers to:
  • Biophysical Profile (BPP): a scoring system that assesses foetal breathing movements, body movements, muscle tone, amniotic fluid, and sometimes foetal heart rate reactivity. A score of 8/8 or 10/10 is reassuring. Scores below 6 trigger further assessment.
  • Amniotic Fluid Index (AFI) : too little fluid (oligohydramnios) can indicate fetal kidney problems or placental insufficiency. Too much (polyhydramnios) can signal gestational diabetes or fetal swallowing difficulties.
  • Doppler Studies : blood flow measurements through the umbilical artery tell us whether the placenta is delivering adequate nutrition and oxygen to the baby. Abnormal Doppler findings are an early warning sign of fetal growth restriction.

Yes. Every obstetric ultrasound in this package is performed by our radiologist on the same equipment used for all Felix Diagnostic scans. Package pricing does not affect scan quality, time allocated, or report detail.

What if my scan findings require additional monitoring beyond the three scans?

Our gynaecologist will recommend additional scans based on clinical findings — gestational diabetes, growth restriction, low-lying placenta, twins, or other findings that warrant closer monitoring. Additional scans are bookable individually. We will never tell you everything is fine when it isn't to avoid recommending a scan outside the package.

Is this package available for twin pregnancies?

Yes — twin pregnancies are monitored using the same ultrasound package, though they typically require more frequent scans than the three included here. Our gynaecologist will assess your specific twin pregnancy type (monochorionic or dichorionic) at the first scan and advise on the recommended monitoring schedule for your case.

Do I need to fast before pregnancy ultrasounds?

No fasting required for obstetric ultrasounds. A moderately full bladder is recommended — drink 2–3 glasses of water about an hour before your appointment. For any blood tests that may be recommended alongside your scans, fasting requirements will be confirmed when you book.
